You certify that all information provided in this application is true, accurate, and complete. You authorize Perfect Optical Corporation and/or Perfect Optical Lab Corporation, as well as their agents or representatives, to obtain and verify credit and financial information from credit reporting agencies, financial institutions, trade references, or other sources deemed necessary to assess creditworthiness.

You, both individually and on behalf of any business entity associated with this application, unconditionally and irrevocably guarantee full and prompt payment of all obligations, debts, and liabilities, whether existing or incurred in the future, to Perfect Optical Corporation and/or Perfect Optical Lab Corporation. This guarantee remains in effect until all outstanding amounts are paid in full. You waive any requirement that Perfect Optical Corporation and/or Perfect Optical Lab Corporation first attempt to collect from the business entity before seeking payment from you personally.

In the event of default, you agree to pay all reasonable costs of collection, including but not limited to attorneys' fees, court costs, and collection agency fees, in addition to the principal balance owed.

You understand that your electronic signature is legally binding and has the same force and effect as a handwritten signature.

This agreement is governed by and construed in accordance with the laws of the State of New Jersey. Any disputes arising from this agreement will be resolved exclusively in a court of competent jurisdiction in the State of New Jersey.

Terms: Payment is due by the 15th of each month. Payments after the 20th may be subject to a 2% late fee. Balances over 30 days from statement date are considered past due and may be subject to a finance charge of 1.33% and may be subject to service interruption without notice.
